Transitions Customized Solution Comes to Collingswood

Typical residents need observation for new clinical situations such as medications, therapies and devices; rehabilitation to regain skills to manage living at home; extra support to gain confidence before returning home; time to allow the implementation of support structures at home; and further stabilization. While the environment is less restrictive, it continues to support overall recovery and wellness.
