Bristol Glen Achieves Two Distinctions from Atlantic Health Systems
UMC at Bristol Glen has been recognized as an official Atlantic Health Systems’ CHF/COPD Clinical Pathway Preferred Provider. The goal of the pathway is to standardize care and improve resident outcomes for those with congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD), two common and serious conditions among older adults.
Preferred provider status is based on evidence-based guidelines and criteria such as hospital readmission rates, length of stay, and clinical complexity. Bristol Glen had zero hospital readmissions among their 32 participating residents during the evaluation period, demonstrating their high quality of care and efficiency in healthcare delivery.
Additionally, Bristol Glen has also been recertified as a member of Atlantic Health System’s High Performing Network, a select group of skilled nursing facilities that meet or exceed value, quality, and collaboration score performance. The network targets Atlantic Health System’s legacy hospitals, including Newton Medical Center, the nearest hospital to Bristol Glen. Atlantic Health also considers skilled nursing providers and senior communities that support and accommodate placement for patients with behavioral health challenges and pending Medicaid status.
Bristol Glen is one of just 36 care providers to have High Performing status with Atlantic Health Systems, an especially notable accomplishment for a Continuing Care Retirement Community (CCRC) offering a full spectrum of services.
