Our staffing model supports high quality with the use of RNs, not LPNs, and a person-centered model for providing care and services.
Resident satisfaction is an important component of how we track our effectiveness. We want to be sure you have a positive experience in our sub-acute neighborhoods. All sub-acute residents are given a satisfaction survey upon discharge with a stamped/addressed envelope. Ratings measure the following: overall satisfaction, care received, services rendered, and discharge planning. Metrics are scored on a five point scale with one being the lowest and five being the highest. United Methodist Communities consistently ranks high in all areas with average range of 4-5 scores.
We are proud of our unique therapeutic programs.
- Our Community Care Transitions Program follows residents for 30 days after discharge from a sub-acute stay at United Methodist Communities. This free program includes one home visit within 24-48 hours of discharge and three subsequent weekly calls, ensuring the home environment is safe and the resident is able to manage their needs.
- Our Care Transitions Program to Assisted Living targets residents not needing full sub-acute care but, who are not ready to be safely discharged to home. Transitions bridges recovery in a home-like apartment setting. This short term program, lasting one to six weeks, works towards getting you back home safely.
- We offer in-house respiratory therapy, which is a service not offered in all post- acute settings. This allows patients with chronic obstructive pulmonary disease, congestive heart failure, or other diagnosis in which pulmonary function is impaired an earlier discharge from a hospital.
- Our 30-day hospital readmission rates are substantially below state and national averages.
